       The recipient's name and address

             Whether the contribution is in honor of a special occasion

       Whether the contribution is in memory of someone  and provide name

  Your name as you want it to appear in the note that we send the recipient

             Your address

             Your contact telephone number, email or fax should we have a question

TO ORDER MULTIPLE CARDS ONLINE:

    Click on the blue order online button.  Fill in the appropriate information.  As soon as your check is received the cards will be sent -- a confirmation will be sent to you.

If you want us to mail your multiple card order to you - Please add $4.00 on to your contribution to cover the cost of mailing.

Consider opening an "Account with us"  Send in a contribution to Gloria Fine -- address above - with notation "Tribute Cards" -- The contribution will be deposited immediately and you will be able to draw down from it saving the need to send a check with each contribution.  All you will need to do is indicate with each order how much you want to contribute for that contribution/s or the purchase of multiple cards.
